Registering A New Patient
If you wish to register as a patient with the Practice you can check to see if your address is within the Practice catchment area here: Practice Area Map
To register with Much Wenlock and Cressage Medical Practice you will need to complete the Practice Registration form and the General Medical Services NHS Registration form. Please ensure you complete both of the forms below.

The registration form is also available to collect in Practice from the Reception team.
Once you have completed the registration form, please bring it with you into the Practice to register. You will need to also bring along with you two forms of ID: Something that confirms your address e.g utility bill or council tax bill and a driving licence, passport or Birth Certificate. If you have an NHS medical card, please bring this with you also.
Please note we are unable to process Registration Forms via Email.
Once you have been accepted as a patient, your medical records will be transferred to the practice. Your medical records may take several weeks to arrive at the practice.
We advise all our new patients to make a routine health check a member of our team within the first three months of joining our practice.
Named Accountable GP
On registering with the Practice you will be allocated a named accountable GP who is responsible for your overall care at the practice. Patients are currently allocated to whichever GP has the shorter list of patients. Please ask reception if you wish to know your named GP. Please note this does not affect your ability to see any of the doctors working within the practice. If you have a preference as to which GP will be your named accountable GP please inform reception and we will make all reasonable efforts to accommodate this request.

Temporary visitors and tourists
Anyone, regardless of their country of residence, is entitled to receive NHS primary medical services at a GP practice. This means tourists, or those from abroad visiting friends or family in England, should be treated in the same way as a UK resident. It also means GP practices cannot charge for this.
Patients should be registered as temporary if they intend to reside in the practice area for more than one day but less than three months.
Homeless patients
Homeless patients are entitled to register with a GP using a temporary address, which may be a friend’s address or a day centre. The practice address may also be used to register them.
The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.
If English is not your first language, the practice has access to interpreters. Please inform reception staff and this assistance can be provided.
